Why early CRO involvement can strengthen study design and execution

A well-designed clinical trial starts with clear scientific questions, a statistical approach capable of answering them and an operational strategy that can successfully translate the design into practice.

That is why both biostatistical and clinical operations expertise are valuable well before a study begins, whether the research is industry-sponsored or investigator-initiated.

When brought together early, these perspectives help test assumptions, define meaningful endpoints, assess operational feasibility, anticipate analytical and execution challenges, and keep the evidence generated aligned with the study’s scientific objectives.

This collaborative approach is consistent with ICH E6(R3) Good Clinical Practice guidance, which emphasizes quality in trial design and the involvement of appropriately qualified expertise throughout the trial process [1].

Scientific collaboration across the research lifecycle

Biostatistics can contribute throughout clinical development, from protocol development and study design to sample-size determination, statistical analysis, interpretation and scientific communication.

Clinical operations bring an equally important perspective: whether the study can be executed effectively across sites, countries and participant populations while maintaining consistency with the protocol and scientific intent.

Together, these functions help bridge the gap between how a study is designed and how it is ultimately conducted.

At Seriant, this approach is rooted in more than two decades of scientific and operational collaboration.

Originally established in 2001 as the Montreal Heart Institute Coordinating Centre (MHICC), Seriant has built extensive experience supporting randomized controlled trials, multicentre studies and translational research.

Over that period, our biostatistics and clinical operations teams have contributed to more than 400 publications.

A scientific contribution built over time

Cardiovascular research remains central to Seriant’s scientific heritage, with work spanning heart failure, atrial fibrillation, coronary disease, structural heart disease, transplantation and cardiovascular prevention in journals such as Circulation and the European Heart Journal.

Oncology, infectious disease, medical imaging, anesthesia and precision medicine also feature prominently in Seriant’s publication record, Across a range of research areas, contributions have appearedin leading peer-reviewed journals such as the New England Journal of Medicine, Nature Medicine, JAMA, The Lancet and The Lancet Oncology.

This breadth reflects more than publication volume. It represents sustained collaboration with investigators and multidisciplinary research teams across a wide range of scientific questions and therapeutic areas.

Why integration matters

For clinical trial sponsors, this experience matters because scientific rigor and operational expertise are essential throughout the study lifecycle, from initial design and planning through execution, final analysis and interpretation.

Cross-functional collaboration helps:

  • challenge assumptions that may affect study feasibility or interpretation
  • assess whether study design can be translated effectively into operational execution
  • align endpoints and analysis methods with the scientific question
  • anticipate challenges related to recruitment, data collection, missing data and analysis populations
  • support transparent, prespecified analytical decision-making
  • maintain continuity between the protocol, study execution, database and final analysis

When biostatistics and clinical operations work together from the outset, decisions made during study design can be carried more consistently through execution, data collection, analysis and interpretation.

That continuity can help identify potential issues earlier and reduce avoidable complexity later in the trial.

From study design to scientific evidence

At Seriant, clinical development is approached as an integrated process.

Our clinical operations, data management and biostatistics teams work across the study lifecycle to help connect scientific objectives with operational execution and the evidence ultimately generated.

The goal is not simply to execute a protocol or produce statistical outputs. It is to help sponsors and investigators conduct well-designed, well-executed studies that generate reliable evidence and support informed scientific decision-making.

Transforming lives, one study at a time.
